A combination unit, usually a wall-hung condensing boiler with a built-in or plate heat exchanger for domestic hot water. Builders used them widely in Vaughan townhomes because they save mechanical room space. The tradeoff is that a hot water fault and a heating fault can be the same fault, so losing hot water in January can mean losing your heat too. We treat those as priority calls.
Why does my tankless give me hot, then cold, then hot?
That is the cold water sandwich. The unit fires, you get hot water, a brief interruption in flow drops it below the activation threshold so it stops firing, and the slug of water already in the pipe arrives cold before it restarts. The usual causes are a failing flow sensor or a scaled heat exchanger that cannot modulate properly. Both are repairable.
How often does a tankless unit need descaling?
Roughly annually in this area, and it is not optional maintenance the way it nearly is on a tank. The passages inside a tankless heat exchanger are narrow, so a scale layer that a tank would shrug off restricts flow and drives the unit toward lockout.

At twenty years you are past the point where repair is good value. The anode will be long gone and the lining is the only thing left protecting the shell.
